Exit interviews
You might be doing exit interviews when they leave, but are you getting the real reason they have left /are leaving, or some watered-down, more palatable reason because the person leaving does not want to ‘burn their bridges’? (I know about this because I did this myself).
Getting an independent body to do your exit interviews would increase the likelihood of you getting better feedback so that you can then do something about a problem should there be one.
